Tony Laskowski and Pat Dziak Memorial Scholarship for Physical Therapist Assistant Students

This application is for Fall 2025.

PURPOSE: The purpose of this scholarship is to support 2 COD PTA students demonstrating leadership skills and community service who are in need of assistance to complete the second year of the Physical Therapist Assistant Program to achieve an Associate’s Degree required for licensure application as a Physical Therapist Assistant.

Applicants must have the following criteria:
• Applicant must be beginning their second year in the COD Physical Therapist Assistant Associate of Applied Science program in the Fall term.
• Applicant must submit a personal statement (up to 700 words) detailing their career goals and commitment to physical therapy
